Indications/Uses
CHF, nephrotic syndrome, liver cirrhosis w/ oedema (fluid retention) & ascites (swelling of the abdomen); malignant ascites. Diagnosis & treatment of primary aldosteronism.
Dosage/Direction for Use
Adult CHF 100 mg/day. Maintenance dose: 25-200 mg/day. Max: 400 mg/day. Nephrotic syndrome 100-200 mg/day. Liver cirrhosis w/ fluid retention & swelling of the abdomen 100-400 mg/day. Cancerous disease w/ swelling of the abdomen 100-200 mg/day. Max: 400 mg/day. Diagnosis of primary aldosteronism. Long test: 400 mg/day for 3-4 wk. Short test: 400 mg/day for 4 days. Treatment of primary aldosteronism 100-400 mg/day. Childn Starting dose: 3 mg/kg body wt.
Administration
Should be taken with food.
Contraindications
Hypersensitivity. Diabetes w/ or w/o kidney problems; Addison's disease. Severe or worsening kidney problems; high blood levels of K; urinary retention.
Special Precautions
High K diet. Low blood levels of Na; liver cirrhosis; severe illness. Decreased urine output; too much acid in the body. Inherited disorder of Hb causing skin blisters, abdominal pain & nervous system disorders (porphyria). Abnormal periods or swollen breasts. Sugar intolerance. Monitor levels of body fluid & chemicals in elderly & patients w/ impaired renal or hepatic function. Diabetes. May affect ability to drive or operate machinery. Pregnancy & lactation.
Adverse Reactions
Increase in blood levels of nitrogen or urea especially those w/ impaired kidney function, altered numbers & types of blood cells. Low blood levels of Na, changes in the levels of chemicals in the body. Lack of muscle coordination, drowsiness, dizziness, headache, clumsiness; tiredness; inflammation of the stomach lining, bleeding, stomach or intestinal ulcers, stomach cramps, diarrhoea, being sick; skin rashes including pale or red irregular raised patches w/ severe itching (hives), hair loss; bone softening due to vit D deficiency; acute kidney failure in those who already have impaired kidney function; enlarged breast in men, changes in voice pitch, reduced sexual potency in men, decreased sexual ability, breast tenderness & increased hair growth in females, irregular periods, sweating.
Drug Interactions
Antihypertensives eg, ACE inhibitors (captopril or enalapril) & angiotensin-II receptor antagonists (valsartan or losartan). Chlorpropamide, digoxin, ciclosporin, or tacrolimus, fludrocortisone, warfarin, other K-sparing diuretics, lithium, NSAIDs eg, indomethacin & mefenamic acid; aspirin, K supplements, noradrenaline, anaesth, carbenoxolone.
